The long-term care chain has selected PatientPing, a software vendor, to provide its application that notifies providers in real time when patients receive care throughout a continuum, better enabling healthcare organizations to collaborate and know what care has been delivered to common patients.

HCR ManorCare operates a network of nearly 500 skilled nursing and rehabilitation services, memory care centers, assisted living facilities, outpatient rehabilitation clinics, and hospice and home health agencies.

To succeed in accountable care environment, providers must collaborate more closely on patients being treated by providers throughout the continuum. ManorCare adopted the PatientPing technology because it wants to reduce unnecessary hospital readmissions, improve outcomes and drive savings for hospitals that face reimbursement penalties from insurers because of unnecessary readmissions, its executives say.

To achieve that ManorCare places nurses in hospitals to write discharge plans; if a patient returns to the hospital, those nurses can refer the patients to a nursing home, if possible, reducing readmission rates.
